The community makes all the maps right now. There is only one or two "official maps". Aerowalk is a Quake 1 map, and since Q1 is basically open-source you can make copies with no intellectual property issues. Normally the mapper will contact the original creator of the map and ask for permission to port that map to a game. This is the case with "Toxicity" which was ported to reflex with permission of the original mapper.

Not yet but the outlines (player outlines) are more pronounced if your play on a map with a lightmap built.

Yes the config process is the same, however the in-game menus are alot easier than trying to beat up a config. There are however things you cannot do in the in-game menus that need to be configged.
In regards to tiered armor.
You cant stack armor. The armors are given a value, and you are "at that value" until the armor is shot-off, or you pickup a higher valued armor. So if I pickup a green I am at "green value" and can only pickup shards. If I am at yellow, I cannot pickup a green until the yellow is shot off.
The values for armors should be on the wiki.
